I need default text from the aff_app_submitted_auto_approved under MESSAGING

I was in the process of changing/editing the emails that are sent after someone signs up but when I clicked EDIT it just copied text from my clipboard and I accidentally saved. This email had shortcode that represented their username and password. Can someone help provide me what that default text was that should include their username and password. This is if I have it set to auto approve which I do. Look at the image below please. I deleted this plugin 3 times from FTP and tried to clean out the tables and every time I reinstalled, it still had the text you see below and NOT the default text that was there when I first installed. Any help would be appreciated.
