[Resolved] Form disappeared on registration page

Hi there – I set up the Affiliate plugin some years ago before switching over to Kartra CRM and editing the registration page to include Kartra HTML code (which is now disabled as is my Kartra account). Now, I have cancelled Kartra and would like to switch back over to the regular registration form, but can’t figure out how. I already tried deleting the WP Affiliate Manager plugin, reinstalling and activating it, but cannot get the page to go back to default settings. Can you help? Is there a shortcode solution to this? Thanks in advance.
