registration not working

I am not sure what is going on but after potential affiliates register it goes to a blank page and i am never notified of a registration. I have tried both auto approve and manual and no luck. I am using a shortcode on a custom page.

I also have simple membership installed as well as the integration add on. not sure what exactly the issues is.

path I would like is: affiliates > registration > confirmation(mydashboard) > autoapprove > email affiliate to login and create password > affiliate dash

current path: affiliates > registration > blank page > affiliate registers under membership subscription service plugin

I tried adding free subscription under simple membership that is an affiliate membership(free) but that the sign still only goes under simple membership and not as an affiliate under the WP affiliate manager plugin.

so i understand how the plugin integration works but what I am not able to sort out how when a potential affiliate signups, why cant i see them in the affiliate admin dashboard or get a notification. I ran through the registration and never received the email(set to auto-approve) to finish the signup process and create login credentials.

They register under my affiliate registration page but instead of becoming an affiliate and showing up under my affiliate dashboard they become a simple membership member.

How are they suppose to become an affiliate and get an affiliate login credentials when it never registers in the affiliate system?

UPDATE: i was able to sort out the path issues, not sure if it was related but after updating the mailchimp integration it all started running smoothly.
