required fields error

Hello
just installed your plugin and tested registering – entered all the required fields in correct formats but I keep seeing these messages below:

First Name * is required
Last Name * is required
Phone Number * must be a valid phone number
E-Mail Address * must be a valid e-mail address
Address Line 1 * is required
City * is required
State * must be selected
Zip Code * must be a valid zip code
Country * is required

what’s going on?..

also how can I make the data already entered appear in the fields instead of showing a blank form again – having people restart?
